Graduate Program Update Forms and Plans

These forms allow current students to make changes to their degree plans. Forms include readmission, reinstatement, and leave of absence, as well as changes to majors or catalog year.

ALL of these forms are fillable PDF forms and should be completed and submitted as such. Download the form, SAVE, complete the form and use electronic signature (student), SAVE and submit to gradcollegeforms@kennesaw.edu upon completion.

You will receive a response through your KSU email within 2-3 weeks. 

  • If you were enrolled in graduate classes, but have not enrolled for at least three consecutive semesters, including Summer term, and wish to be readmitted submit this form.

    If you were accepted into a graduate program but did not attend please complete the Update Form through the Office of Graduate Admission.

    If your KSU email is deactivated, we will contact you through your personal email. For readmission only.

    Readmit Deadlines:

    • Fall semesters - July 15th
    • Spring semesters - Dec 1st
    • Summer semesters - April 15th

  • Complete this form if you were academically dismissed and wish to appeal for reinstatement. You will be notified via your KSU email when a decision has been made. 

  • For students experiencing unusual circumstances, a leave of absence can be requested. Note, an approved Leave of Absence is required for students enrolled in Thesis or Dissertation hours only. This form requires approval of the Graduate Program Coordinator and The Graduate College. Once your coordinator has approved email to gradcollegeforms@kennesaw.edu

  • Deadlines for Request for Program Changes /Concentration
    • Fall semesters - July 15th  
    • Spring semesters - Dec 1st
    • Summer semesters - April 15th

    If you are currently in the application process and wish to change your application major please email ksugrad@kennesaw.edu.

  • Current students who wish to change from their current catalog year. 

  • All requirements for a master’s degree program must be completed within six years. All requirements for a doctoral degree must be completed within ten years. The Graduate College may grant an extension of time for conditions beyond the student's control. Individual programs may have different requirements for that program's time limit.  Refer to program descriptions in this catalog for additional information.

  • Students may request transfer credit from a regionally accredited institution where graduate coursework was taken. Awarding of transfer credit is at the discretion of the program coordinator of the program and must be initiated by the program coordinator.

  • IMPORTANT: ALL requests for an add-on certificate must be submitted no later than four weeks prior to the beginning of the semester. Please complete the top portion of the form and email to gradcollegeforms@kennesaw.edu processing.

Thesis and Dissertation Forms

We want you to have access to the resources that you need. This section will provide direct access to important forms needed for the successful completion of your graduate and/or doctoral degree program.

Doctoral Forms Use and Instructions

All forms are fillable PDFs and should be completed as such. Download the form, SAVE, Complete form, SAVE with your name in title. You are strongly encouraged to use DocuSign for signatures. The Graduate Approval should be the last signature requested. When adding for graduate approval use the following:

  • Request for Approval of Dissertation Committee. Must be completed in the semester in which the committee is formed. The Chair of the committee must be KSU Graduate Faculty associated with your academic program. Doctoral Dissertation Committees usually consist of five member committees, at least three of which should be KSU Graduate Faculty.

  • Dissertation Proposal Approval. Must be completed and submitted by the end of the semester the dissertation has been approved.

  • Apply for Admission to Candidacy. Must be completed by the end of the semester after comprehensive exams have been passed.

  • Final Submission
    Must be completed once the final version of the dissertation is ready to be submitted for publication in the Digital Commons by Final Grades Due Date. Follow the directions at the Digital Commons to upload your dissertation. The Digital Commons will notify The Graduate College when your submission is complete.

  • Use this form to:

    Request for Approval of Thesis Committee

    • Must be completed early in the program, once committee members are identified.
      Form must be completed in the semester in which the committee is formed.
    • The Chair of the committee must be KSU Graduate Faculty associated with your academic program.
    • Thesis Committees are normally three member committees, at least two of which should be KSU Graduate Faculty.
  • Must be completed and submitted by the end of the semester the dissertation has been approved.

  • Must be completed after the dissertation has been successfully defended, and must be submitted a week before grades are due (to ensure adequate time to enter it into the system).

    Final Submissions 
    Must be completed when the final version of the graduate school thesis is ready to be submitted for publication in the Digital Commons. Follow the directions at the Digital Commons to upload your thesis. The Digital Commons will notify The Graduate College when your submission is complete.
